How Can I Replace My Lost or Damaged OSHA Card?

If your OSHA 10 or 30-hour OSHA wallet card has been lost or damaged, you may be eligible to purchase a replacement card. However, there are a few details to know about OSHA’s policy on replacement cards to determine if you are eligible to purchase a new card, and if so, the process you must follow to obtain the new card.

First of all, be aware that OSHA policy only allows wallet cards to be replaced if your 10 or 30-hour course was completed no more than five (5) years prior to when make your request for a replacement card. That is because trainers and online providers are only required to maintain their records for five years.

Also, be aware that OSHA policy only allows a student to request a replacement card one time for a class they previously took. So if you already requested and received a replacement card for a course you took, you will not be able to get a second replacement card.

Next, you must determine to whom you must make your request for a replacement card. And that will depend on whether you took the OSHA 10 or 30-hour course online or in a live class.

OSHA does not maintain a central data base of OSHA classes and students. The records of your training are with the person who conducted your live class, or the online provider for your course. All requests for replacement cards must be made thorough your original OSHA trainer or online course provider.

If you took your OSHA 10 or 30-hour course through one of the OSHA-authorized online training providers, you must contact that online provider directly to purchase a new card. The online provider will check their records to confirm that you completed their course and meet the five year time requirement, and if so, they will order you a new card.

If your OSHA 10 or 30-hour class was conducted live by an OSHA-authorized Outreach trainer, you must contact your trainer directly to request a new card. The trainer will check their records to confirm that you completed their course and meet the five year time requirement, and if so, they will order you a new card.

If you do not remember the name of your trainer, here are some suggestions:

  • Check with your employer or union, if they arranged your past class, to see if they know the trainer’s name and contact info;
  • Check with someone else who was in your class, and look at their OSHA card for the trainer name;
  • Check your credit card or checking account info if you payed for your own course to find the name of the provider;
  • Check old emails to look for related messages.

Since the trainer or online provider has to pay a fee to obtain your replacement card, OSHA policy allows them to charge you, the student, a fee to obtain and send you a replacement card. The fee the student can be charged for the replacement card is determined by the trainer or online provider, and can be costly.

If you are unable to locate your past trainer or online provider, you will have to retake the course to get a new OSHA wallet card.
